CHAPTER 2 Amendment of Commission Regulation on continuing airworthiness

Amendment of the Appendices to Annex 4 to Commission Regulation (EU) No 1321/2014

283.—(1) The Appendices to Annex 4 are amended as follows.

(2) In Appendix 2—

(a) in the title, for “EASA Form 11” substitute “CAA Form 11”;

(b) on the first page of the form—

(i) for “[Member State (*)] A Member of the European Union (**)” substitute “United Kingdom”;

(ii) for “[Member State code (*)]” substitute “United Kingdom”;

(iii) in the first place the words occur, for “[Competent authority of the Member State (*)]” substitute “Civil Aviation Authority”;

(iv) for “For the competent authority: [Competent authority of the Member State (*)]” substitute “For the Civil Aviation Authority”;

(v) for “EASA Form 11 Issue 5” substitute “CAA Form 11 Issue 1”;

(vi) omit the notes at “(*)” and “(**)”;

(c) on the second page of the form—

(i) for “[Member State code (*)]” substitute “United Kingdom”;

(ii) for “For the competent authority: [Competent authority of the Member State (*)]” substitute “For the Civil Aviation Authority”;

(iii) for “EASA Form 11 Issue 5” substitute “CAA Form 11 Issue 1”;

(iv) omit the note at “(*)”.

(3) In Appendix 3—

(a) in the heading, for “EASA Forms” substitute “CAA Forms”;

(b) in the first form—

(i) in both places the words occur, for “[Member State code (*)]” substitute “United Kingdom”;

(ii) for “EASA Form 148 Issue 2” substitute “CAA Form 148 Issue 1”;

(iii) omit the note at “(*)”;

(c) in the second form—

(i) in both places the words occur, for “[Member State code (*)]” substitute “United Kingdom”;

(ii) for “EASA Form 149 Issue 3” substitute “CAA Form 149 Issue 1”;

(iii) omit the note at “(*)”.
