a
for “services-provider”, in each place it appears, substitute “responsible person”,

in subsection (2), before paragraph (a) insert—

the National Health Service Commissioning Board;

a clinical commissioning group;
c
in that subsection, omit paragraph (c), and
d
in the title, for “services-providers” substitute “responsible persons”.
