(a) at the beginning insert “the National Health Service Commissioning Board, a clinical commissioning group,”,

(b) omit “Strategic Health Authority,”,

(c) for “, Special Health Authority” substitute “or Special Health Authority”, and

(d) omit “or Primary Care Trust”.
