Amendment of Appendix 11 to Annex 1 to Commission Regulation (EU) No 748/2012

235.  Appendix 11 (EASA Form 65) is amended as follows—

(a) in the heading, for “EASA Form 65” substitute “CAA Form 65”;

(b) for “[Member State] (*)” substitute “United Kingdom”;

(c) omit “A Member of the European Union (**)”;

(d) for “[Member State code (**)]” substitute “United Kingdom”;

(e) for “competent authority: [competent authority identification (*)2]” substitute “CAA”;

(f) for “EASA Form 65, Issue 2” substitute “CAA Form 65, Issue 1”;

(g) omit the notes at “(*)” and “(**)”.
