3.—(1) Ystod oedran arfaethedig y disgyblion.
(2) Uchafswm arfaethedig y disgyblion.
(3) P'un a yw'r ysgol ar gyfer disgyblion gwrywaidd, neu ddisgyblion benywaidd neu'r ddau.
(4) P'un a yw'r ysgol yn darparu llety byrddio ar gyfer disgyblion.
(5) P'un a yw'r ysgol yn derbyn disgyblion ag anghenion addysgol arbennig.
(6) P'un a fydd yr ysgol yn darparu'n gyfan gwbl neu'n bennaf ar gyfer disgyblion ag anghenion addysgol arbennig a'r math o anhawster dysgu y darperir ar ei gyfer.
(7) P'un a fydd yr ysgol yn darparu gofal dydd o fewn ystyr paragraff 1(2) o Atodlen 9A i Ddeddf Plant 1989(1) ar gyfer unrhyw blentyn y gofalir amdano yn yr ysgol.
(8) Plan yn dangos gweddlun y tir a'r adeiladau a'r llety.
(9) Cynlluniau cwricwlwm manwl, cynlluniau gwaith a gweithdrefnau asesu disgyblion.
(10) Copïau o'r polisïau ysgrifenedig sy'n ofynnol gan reoliad 3(2) o'r Atodlen i Reoliadau Ysgol Annibynnol (Safonau) (Cymru) 2003(2).
(11) Copi o'r drefn gwyno y mae paragraff 7 o'r Atodlen i Reoliadau Safonau Ysgol Annibynnol (Cymru) 2003 yn ei gwneud yn ofynnol.
(12) P'un a yw'r perchennog yn bwriadu darparu llety byrddio i unrhyw blentyn yn yr ysgol (neu yn rhywle arall yn unol â threfniadau a wnaed ganddo) am fwy na 295 diwrnod mewn unrhyw flwyddyn.
(13) Anian grefyddol yr ysgol, os oes un.
(14) P'un a yw tir ac adeiladau'r ysgol, gan gynnwys llety byrddio, mewn dau leoliad neu fwy, ac os felly, cyfeiriad pob un o'r lleoliadau.
(15) Os yw'r ysgol yn elusen, neu'n cael ei rhedeg gan elusen, enw'r elusen honno a'i Rhif cofrestru.
(16) Copi o'r Dystysgrif Ddatgelu ar y lefel briodol a ddyroddir gan y Swyddfa Cofnodion Troseddol yn cadarnhau bod y perchennog yn addas i weithio gyda phlant.
(17) Copi o asesiad risgiau'r ysgol o dan reoliad 3 o Reoliadau Rheoli Iechyd a Diogelwch yn y Gwaith 1999(3) i'r graddau y mae'n berthnasol i rwymedigaethau o dan Ran II o Reoliadau Rhagofalon Tân (y Gweithle) 1997(4)).
